Dependent Reinstatement following Dependent Eligibility Verification Project
Submit Dependent documents via one of the below methods to have eligibility reinstated retroactively to the termination date:
- Fax to (602) 324-0555
- Submit through Secure Portal at www.ssatpa.com
- Mail to: Teamsters Western Region and New Jersey Health Care Fund

Spouse; submit a copy of the marriage certificate and a copy of the front page of your most recent Federal Tax Return (income information can be redacted or hidden).
Child(ren); submit a copy of the birth certificate or adoption certificate. If this document does not list the primary insured as the parent, additional information will be required to verify the child’s relationship.
Disabled adult dependent child; submit a copy of the Social Security Administration Security Statement and proof that you provide financial support for the child
